The 3-640440-5 from TE Connectivity is a MTA-100, single row, 5 position wire to board receptacle connector assembly with strain relief.  It has closed end connector design and 90° cable exit angle. This wire to board connector is used with MTA.100 headers and MTA.100 IDC posted connectors. It is approved by UL E28476 and CSA standards.
